SEAMLESS CARE
An Interprofessional Education Project for
Innovative Team-Based Transition Care

September, 2007
Between January and April 2007, 37 students on 9 interprofessional teams worked together collaboratively at a Seamless Care clinical site under the guidance of their clinical preceptors.

We are currently analyzing data gathered from students, preceptors, and patients about this research project. This analysis will be used in supporting the benefits of interprofessional education and inform change and future development.

This phase of the project will see members of the research team, staff, steering committee, and project participants present at two international conferences. A variety of papers will also be published in the upcoming year.
